CRUSH COVID RI - Privacy Policy

CRUSH COVID RI, its Developer1 and affiliates (collectively, “We”) are committed to protecting and preserving your privacy.  This Privacy Policy (“Policy”), together with our Terms and Conditions of Use (“Terms”), and any other underlying agreements or terms, sets out the basis on which any data we collect from you, or that you provide to us, may be processed by us.  By using our mobile application (“Application”), you are accepting the practices described in this Policy. This Policy also describes the manner in which we process, use and disclose information that you provide to us when you use the Application.

By using this Application, you consent to the use of the information that you provide to us in accordance with this Privacy Policy.  We update this Policy from time to time with or without notice and we recommend that you review this Policy regularly.  You will be notified of any material changes to this Policy through a notice within the Application. Your continued use of the Application constitutes your agreement that your information may be used in accordance with the updated Policy.  If you do not agree with the changes, you must stop using the Application immediately and delete it from your device and notify the Developer as listed in the defined terms section of Terms and Conditions that you do not want your information used in accordance with the changes. 

Mobile Application Privacy Notice

Please read this Policy carefully to fully understand how we collect, share, and protect your information.  This Policy is incorporated into and is a part of the Terms and Conditions of Use for this Application.  Access the Terms and Conditions of Use.

By downloading, accessing and using the Application, you agree that you have read and understand this Privacy Policy and that you accept and consent to the privacy practices, uses, and disclosures of your information as described herein. 

In order to deliver personalized and relevant information to you through the Application, we collect certain information from Users. 

Information We Collect

By operating the Application, we may collect and process the following data about you:

  1. Details of your visits to the Application and the resources that you access, including, but not limited to, news posts, the date and time you accessed or left the Application, or notifications you received through the Application. 
  2. Information that you provide directly to us when downloading or using the Application, or when you opt-in to providing information to us in order to use certain features, such as your name, date of birth, zip code, and symptoms.
  3. Information that you provide directly to us by downloading and using the Application, and if you opt-in to use anonymous features of the app, we use your unique, anonymous Application ID2 to identify your installation of the Application to support the delivery of those services.

Use of Your Information

The information that we collect and store relating to you is primarily used to enable us to provide our products and/or services to you in the best possible manner.  In addition, we may use the information for the following purposes:

  1. To provide you with information relating to our services and/or products and information relating to COVID-19;
  2. To analyze statistical information about the users of the Application;
  3. To monitor and conduct an analysis of the Application traffic and usage patterns;
  4. To analyze trends.

Security

To secure your information, we have put in place commercially suitable physical, electronic, and managerial procedures to safeguard and secure the information we collect on the Application.

Nonetheless, the transmission of information via the internet is not completely secure and therefore we cannot guarantee the security of data sent to us electronically through the Application, and transmission of such data is therefore entirely at your own risk.

When you choose to use the Application to store your health information, such as a vaccination record, the Application has security measures such as encrypting the data on your phone or requiring the input of a passcode to access the information.  However, you acknowledge that you are responsible for keeping your device secure to ensure the security of any data you choose to store in the Application and that any such data you store within the Application is only as secure as your device.

Disclosing Your Information

We will only disclose your information when you have given us permission to share it. For instance, you may consent to share your name, date of birth, and zip code with us in order to access your vaccine record, which will be used to identify your vaccine record in other applications maintained by the Rhode Island Department of Health. We may share your unique, anonymous Application ID with third parties necessary to provide products and/or services to you.  We may also be required to disclose your information to third parties, such as where we are legally required to disclose your information, including, but not limited to, pursuant to a valid subpoena, order, search warrant or other law enforcement request.

Third Party Websites and Mobile Applications

By accessing third party websites or mobile applications through the Application, you are consenting to the terms and privacy policies of those websites and mobile applications.  We do not accept any responsibility or liability for their policies whatsoever as we have no control over them.

Users Only of Legal Age of Majority

The Application is designed and intended for those who have reached 16 years of age. By using the Application, you affirm that you are at least 16 years of age or older.  We are not liable for any damages that may result from a user’s misrepresentation of age.

No one under age 16 is authorized to submit or post any information, including personally identifying information, n the Application. Under no circumstances may anyone under age 16 use the Application. 

By entering information of another person into the Application, you are affirming that you have the authority to enter that information.  For example, a parent or guardian entering information relating to a minor is expressly representing that they have the legal right to access and enter that information.

2Each time the Application is installed, it is assigned a unique, anonymous ID.  This ID is not in any way linked to a User’s name or other personal identifying information. If the Application is uninstalled and reinstalled, a new Application ID is assigned.
